To improve health equity we must understand what is happening in individuals' day-to-day lives outside of the healthcare they are receiving. Those circumstances, referred to as the social determinants of health (SDOH), significantly affect individual and community health and healthcare outcomes. And, when SDOH data is appropriately collected, used, and securely shared, the entire healthcare team can gain insight into various elements that make up a person’s medical and non-medical story, allowing them to work on improving that individual's overall health and well-being.
During this session, speakers will share how they connect the dots between SDOH Data and health outcomes. They will share best practices, tactics, and strategies from their organizations and highlight the power collecting, using, and sharing SDOH data can have in improving health and healthcare outcomes. Speakers will also share the opportunities and challenges facing health information professionals as their organizations use SDOH data.
